Factors Associated with Failure following Hip Arthroscopy for FAI in Patients over 40

Hip arthroscopy is considered one option in preserving the hip. Previous studies have shown age may be a predictor of failure; however, it unclear what factors lead to failure older patient. The purpose this study was determine associated with conversion total hip arthroplasty patient over 40 undergoing arthroscopy. 268 patients were identified who years age, had primary for FAI labral repair. All minimum joint space >2mm. There 144 females and124 males, an average 49±6. Hips evaluated using modified Harris score(mHHS), Outcome Score (HOS), WOMAC and satisfaction (1 10; 10=very satisfied). Conversion THA confirmed by patients. 87 underwent THA. Patients converted significantly (50 vs 48;p=0.001), higher Tonnis angle (10.7 6.8;p=0.016), lower lateral center edge (34 37;p=0.044), larger alpha (77 70;p=0.028). following preoperative subjective complaints THA: stiffness interfering activity 3.7 [95%CI, 2-7] time more likely convert THA(p<0.001); weakness 2.7[95%CI,1.5-5] times THA(p=0.001) loss motion 1.9{95%CI,1.1 3.2] At arthroscopy, presence femoral cyst not THR, but acetabular 3.0 [95%CI,1.6-5.4] microfracture 10 6-18] THR. Using regression analysis, weakness, microfracture, independent predictors (p<0.001, R2=0.408). Of did THA, at 7-year follow-up (5 11 years), mean SF-12 PCS 51.7±9, mHHS 85±17, HOS ADL 89±15. median 5 [95%CI 8.7-12.9] outcome 8-9]. HOS-ADL PASS achieved 75% Factors THR include symptoms stiffness, motion, cysts Those do require excellent outcomes high satisfaction.
